ABSTRACT

Objective To explore the temporal and spatial distribution characteristics of air pollutants PM2.5, PM10, SO2, CO, and NO2, and their effects on acute cerebrovascular diseases in Jining City. Methods The data of patients with acute cerebrovascular disease treated in a 3A hospital in Jining from October 1, 2017, to November 31, 2019, were retrospectively collected. Combined with the air pollution data of 29 air quality monitoring stations in Jining City, the Kriging interpolation model was used to analyze the overall situation of air pollution in Jining. On this basis, the relationship between air pollution and acute cerebrovascular diseases in Jining City was analyzed. Results In Jining City, the incidence of acute cerebrovascular disease in male was higher than that in female, and the incidence in rural areas was significantly higher than that in urban areas. The spatial distribution showed a trend of gradual accumulation from southeast to northwest. The daily average concentrations of PM2.5 and PM10 were higher in winter and spring than in summer and autumn. The results of Kriging interpolation analysis showed that the concentrations of these air pollutants formed aggregation points in varying degrees. The spatial distribution of acute cerebrovascular disease patients in Jining City was highly consistent with the spatial distribution of air pollutant concentrations. Spearman correlation analysis showed that CO, SO2, and NO2 were positively correlated with the incidence of acute cerebrovascular disease, while the correlation between PM2.5 and PM10 and the incidence of acute cerebrovascular disease was not significant. Conclusion Some air pollutants such as CO, SO2, and NO2 have a positive correlation with the incidence of acute cerebrovascular disease, and the prevalence has a certain population and regional distribution. In the future work of cerebrovascular disease prevention, personal protection should be done according to local conditions and living environment of specific people.

ABSTRACT

Objective:To construct a nomogram model for predicting the progression-free survival (PFS) of patients with positive Rictor protein tested in lesion tissues of D2 + radical gastric adenocarcinoma resection and to analyze its predictive value. Methods:The tissue samples of 1 366 gastric adenocarcinoma patients who underwent radical resection in Shanxi Province Cancer Hospital from May 2005 to December 2020 were retrospectively collected, and the Rictor protein expression was detected by using the immunohistochemical SP method. The 676 Rictor-positive cases were grouped in a 7∶3 ratio by simple randomization, including 496 cases in the training cohort and 180 cases in the validation cohort. The correlation of Rictor protein expression and other clinicopathological factors with PFS of Rictor-positive patients was analyzed by using a multifactorial Cox proportional risk model to determine the independent influencing factors of PFS. The nomogram for predicting the 3-year and 5-year PFS rates of patients with gastric adenocarcinoma was constructed based on the independent influencing factors of PFS. The constructed nomogram was bootstrapped with 1 000 resamplings for internal validation to test the accuracy of the prediction model. The internal and external predictive efficacy of the model was further assessed by calibration curves, area under the curve (AUC) of time-dependent receiver operating characteristic (ROC) and decision curve analysis (DCA). The nomogram model was applied to score the PFS of 496 cases in the training cohort, and the X-tile software was used to obtain the best cut-off value for the score. The overall cohort, training cohort and validation cohort cases were divided into low-risk group (≤ best cut-off value) and high-risk group (> best cut-off value) according to the best cut-off value, and the Kaplan-Meier method was used to analyze the difference in PFS between the low-risk group and high-risk group. Results:Multifactorial Cox regression analysis showed that gender, age, pT stage, number of positive lymph nodes, neural invasion, tumor longest diameter, omental invasion, Clavien-Dindo classification of postoperative complications, and CGA expression were independent influencing factors for PFS of the training cohort with Rictor-positive gastric adenocarcinoma. The nomogram for predicting the 3-year and 5-year PFS rates of patients with Rictor-positive gastric adenocarcinoma was constructed based on the above indicators. The calibration curve for internal validation and external validation showed good agreement between the prediction of nomogram and actual PFS. The time-ROC curve showed that the AUC of the internally validated and externally validated models for predicting the 3-year PFS rate was 0.834 (95% CI 0.746-0.823) and 0.799 (95% CI 0.699-0.868), and the AUC for predicting the 5-year PFS rate was 0.817 (95% CI 0.718-0.821) and 0.795 (95% CI 0.675-0.895). The C index of the model for overall prediction was 0.795 (95% CI 0.764-0.825), which was better than the 8th edition of American Joint Committee on Cancer (AJCC) TNM staging [0.693 (95% CI 0.662-0.723)]; the external validation DCA showed that the C index of the model for prediction was 0.769 (95% CI 0.718-0.821). The X-tile software analysis showed that the best cut-off value for the PFS score of the training cohort model was 265.08, with 457, 337 and 120 cases in the low-risk group and 219, 159 and 60 cases in the high-risk group for the overall cohort, training cohort and validation cohort, respectively. Kaplan-Meier survival analysis showed that the median PFS time was not reached in the low-risk group for the overall cohort, training cohort and validation cohort, and the median PFS time was 24, 24 and 28 months in the high-risk group, and there were statistical differences in PFS between the low-risk and high-risk groups for each cohort (all P < 0.001). Conclusions:For the first time, a nomogram model for PFS prediction in gastric adenocarcinoma patients with Rictor-positive expression is successfully constructed, which could better distinguish between patients with low-risk and high-risk of PFS. For high-risk patients with Rictor-positive gastric adenocarcinoma, in addition to controlling tumor metastasis and postoperative complications, attention should be paid to the targeted therapy for positive expression of Rictor.

ABSTRACT

Objective:To investigate the predictive value of the ischemic stroke predictive risk score (iScore) and serum homocysteine (Hcy) for early neurological deterioration (END) in patients with acute ischemic stroke. Methods:Patients with acute ischemic stroke admitted to the Affiliated Hospital of Jining Medical University from July 2018 to June 2020 were enrolled retrospectively. END was defined as the National Institutes of Health Stroke Scale (NIHSS) score within 7 d after admission increased by ≥2 from the baseline. Multivariate logistic regression analysis was used to determine the independent correlations of iScore and serum Hcy level with END, and then the receiver operating characteristics (ROC) curve was used to evaluate the individual and combined predictive values of iScore and serum Hcy for END. Results:A total of 398 patients with acute ischemic stroke were enrolled, including 241 (60.6%) males, aged 65.02±12.17 years. The baseline NIHSS score was 12.15±5.67 and iScore was 124.58±37.51, and 103 patients (25.9%) developed END. Univariate analysis showed that there were significant differences in atrial fibrillation, fasting blood glucose, serum Hcy, stroke etiology type (large artery atherosclerosis and small artery occlusion), baseline NIHSS score and iScore between the END group and the non-END group (all P<0.05). Multivariate logistic regression analysis showed that after adjusting for age, atrial fibrillation, fasting blood glucose, and stroke etiology type, the iScore (odds ratio [ OR] 1.016, 95% confidence interval [ CI] 1.009-1.040; P=0.004), serum Hcy ( OR 1.191, 95% CI 1.075-1.588; P<0.001) and baseline NIHSS score ( OR 1.289, 95% CI 1.101-1.613; P=0.023) had significant independent correlation with END. ROC curve analysis showed that the area under the curve of iScore combined with serum Hcy for predicting END was 0.859 (95% CI 0.820-0.898; P<0.001), which was significantly higher than that of iScore or serum Hcy alone ( P<0.001). The sensitivity and specificity of combined prediction were 81.55% and 85.76%, respectively. Conclusion:The iScore combined with serum Hcy has higher predictive value for END in patients with acute ischemic stroke.

ABSTRACT

Objective@#To investigate the expression of INPP4B in gastric cancer and its relationship with clinicopathological features and prognosis.@*Methods@#The expressions of INPP4B mRNA in fresh cancer tissues of 36 patients with gastric cancer and the paracancerous normal gastric mucosa tissues in the Affiliated Cancer Hospital of Shanxi Medical University between July 2014 and December 2014 were detected by using real-time quantitative polymerase chain reaction (RT-qPCR). The expressions of INPP4B protein and its downstream molecule phosphorylation AKT (p-AKT) in paraffin-embedded tumor tissues and the corresponding margin tissues of 49 gastric cancer patients between January 2010 and December 2010 were detected by using immunohistochemistry. The relationship between the expression of INPP4B and clinicopathological features and prognosis was analyzed.@*Results@#RT-qPCR results showed that the expression level of INPP4B mRNA was 0.21±0.04 compared with adjacent cancer normal tissues (t = -2.208, P < 0.05). Immunohistochemistry showed that INPP4B protein was highly expressed in normal margin tissues and lowly expressed in tumor tissues. There was a statistical difference in the positive intensity score between the two groups (u = 4.70, P < 0.01). However, p-AKT protein was overexpressed in tumor tissues and underexpressed in normal margin tissues. There was a statistical difference in the positive intensity score between the two groups (u = 5.77, P < 0.01). The expression of INPP4B and p-AKT protein was negatively correlated (r = -0.644, P < 0.01). The positive expression rate of INPP4B protein in gastric cancer patients was 34.7% (17/49). There were no statistical differences of the positive expression rate of INPP4B protein in gender, age, pathological type, depth of invasion and lymph node metastasis (all P > 0.05). ABSTRACT

BACKGROUND:Intervertebral disc degeneration is the pathological basis of degenerative spinal diseases. Studies on the influentialfactors of intervertebral disc degeneration contribute to the prevention and treatment of degenerative spinal disease. OBJECTIVE:To observe the growth and proliferation of nucleus pulposus cels isolated by trypsin plus type II colagenase digestion in complete medium with different glucose concentrations, exploring the optimal glucose concentration for growth of nucleus pulposus cels. METHODS:Nucleus pulposus cels isolated and cultured by trypsin plus type II colagenase digestion method were observed under an inverted microscope, and thecelnumber was counted. Morphology of nucleus pulposus cels was observed afterhematoxylin-eosinstaining and toluidine blue staining. Colagen type II immunoreactivity was detected by immunohistochemical staining combined with immunofluorescent staining.Nucleus pulposuscels were incubated in complete medium containing various glucose concentrations (0, 6.25, 12.5, 17.5, and 25 mmol/L) for 24 hours, and then cel proliferation and apoptosis were determined. RESULTS AND CONCLUSION:The stained nucleus pulposus cels showed polygonal and short spindle, with one or two nuclei. Celularpseudopod appeared gradualy and then became slim with increased passage numbers. The isolated and cultured nucleus pulposus cels positively expressed colagen type II and aggrecanProliferative activity of nucleus pulposus cels cultured in medium with 17.5 mmol/L glucose was significantly higher than that in medium with 0 and 25 mmol/L glucose (P< 0.05 orP< 0.01). There wasno significant differencein cel apoptosis between these groups except for 0 mmol/L glucose (P<0.05). These results confirm that a large number of nucleus pulposus celscan beharvested by trypsin plustype II colagenase digestion and the optimal glucose concentration is 17.5 mmol/L.

ABSTRACT

ObjectiveTo investigate the expression and the clinical significance of diacylglycerol kinase α (DGKα) and protein kinase C (PKC) in human hepatocellular carcinoma (HCC).MethodsDGKα and PKC expressions in the samples from 60 pathologically confirmed HCC patients were analyzed by immunohistochemistry. The relationship between DGKo expression and clinical pathology factors was analyzed.ResultsThe expression positive rates of DGKo and PKC were highest in normal liver tissues [90.0% (9/10) and 100.0% (10/10)].The positive rates were 81.7 % (49/60) and 71.7 % (43/60) in HCC tissues, respectively, and were 58.3 % (35/60) and 61.7 % (37/60) in carcinoma adjacent tissues, respectively. In three liver tissues, the positive rates of DGKα and PKC were significantly different (P <0.05).The location of both kinases in hepatocytes translocated from cytoplasm/nucleus to membrane. The expressions of DGKα and PKC were positively correlation(r =0.495, P < 0.05), The DGKα expression was correlated to differentiation type, portal venous tumor thrombus and TNM staging(all P < 0.05).ConclusionDGKa is expressed with high activity in advanced stage and poorly differentiated HCC. It may be promote the pathological process of HCC.

ABSTRACT

Objective To retrospectively evaluate and analyze the clinical effect of posterior pedicle subtraction osteotomy in treating chronic, posttraumatic thoracolumbar kyphosis. Methods Nineteen patients (11 males and 8 females) with chronic, posttraumatic thoracolumbar kyphosis were corrected surgically. The patients were at age range of 29-61 years (mean 42 years). Preoperative kyphosis Cobb angle ranged from 31° to 63° (mean 47°) and trauma history ranged from 8 months to 63 months (mean 29 months). All patients were treated with pedicle subtraction osteotomy according to the size of Cobb angle, extent of spinal stenosis and source of compression. Results Sagittal alignment was improved to average 40.2°, with a correction rate of 85.8%. Two patients developed postoperative leakage of cerebrospinal fluid. Among them, one was combined with encephalic infection and cured with active treatment, and the other developed postoperative wound infection, which were treated conservatively with antibiotics and local wound care. There were no other severe complications. The average follow-up period was 15 months (range 6-41 months). At the last follow-up, clinical symptoms and neurological function were improved significantly. Neither loss of correction nor failure of internal fixators was observed. X-ray and dynamic X-ray films showed a 100% fusion in all patients. Conclusions The single-stage posterior pedicle subtraction osteotomy is a safe and effective procedure for correction of posttraumatic thoracolumbar kyphosis. It is possible and safe to obtain a correction within 55° on single segment by this technique.

ABSTRACT

Objective To investigate the expression and distribution of Diacylglycerol Kinase α (DGKα) mRNA in human hepatocellular carcinoma (HCC),and to explore the function of DGKα in the metastasis of hepatocellular carcinoma. Methods Tissues from 30 cases of HCC and 5 normal liver tissues were collected immediately after surgical resection.Semi-quantitative RT-PCR and in situ hybridization were used to detect the expression levels and distribution of DGKα mRNA,respectively. Results Semi-quantitative RT-PCR showed that the expression level of DGKα mRNA in HCC (0.798±0.317) and normal tissues (0.908±0.425) was significantly higher than those in carcinoma adjacent tissue with cirrhosis (0.205±0.102,P<0.05).In situ hybridization demonstrated that the number of DGKα mRNA positive hepatocytes in HCC [(57.6±6.3)/mm2] and normal tissues [69.8±8.7)/mm2] was significantly higher than those in carcinoma adjacent tissue with cirrhosis [(26.3±4.9)/mm2,P<0.05]; DGKα mRNA was expressed in the cytoplasm of hepatocytes in HCC and carcinoma adjacent tissue with cirrhosis, and in the nuclear of hepatocytes in normal tissues. Conclusion The present study suggests that DGKa may play important roles in carcinogenesis and progressing of HCC.

ABSTRACT

Objective To evaluate clinical significance of the CEA, CA19-9, CA242, AFP, CA724,SCC, TPA, TPS combined detection for the clinical diagnosis of gastrointestial cancer. Methods 373 cases of upper digestive tract malignant tumor patient and 50 cases of healthy serum sample were detected for eight markers, evaluating the relationship between the level of markers and illness. Results 8 marker diagnostic positive rates were: CEA 26.80 %, CA19-9 27.34 %, CA242 34.14 %, AFP 2.84 %, SCC 19.72 %, CA724 12.13 %, TPA 34.15 %, TPS 30.89 %. The total positive rate of joint detection of 8 markers was 89.05 %.Preoperative CEA, CA242, SCC-positive patients had a shorter survival time. CA242 and CA19-9 levels with upper gastrointestinal malignancies were positively correlated. Conclusion The combined detection of a number of markers in different pathological type and clinical staging is statistically significant, and the positive rate is much higher than any single marker test results. CA19-9 and CA242 may be the best combination of monitoring the disease. CA724, CA242 and SCC can be as adverse prognostic indicators for cardia, stomach, esophageal malignant tumors, respectively.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effect of cartilage-derived growth factor (CDGF) on cultured rabbit chondrocytes, and the relation between CDGF, insulin-like growth factor-I (IGF-I), and proliferation and metabolism of chondrocytes.</p><p><b>METHODS</b>CDGF was extracted from chicken xiphoid in our laboratory. Chondrocytes were isolated from 3-week-old New Zealand male rabbits, and cultured in Ham's F-12 medium containing 10% fetal calf serum. The chondrocyts of second generation were treated with CDGF and/or IGF-I of different concentrations. With chloramine T method and MTT method, we compared the content of hydroxyproline in Ham's F-12 medium and observed proliferation and energy synthesis of chondrocytes.</p><p><b>RESULTS</b>CDGF and IGF-I both stimulated the proliferation and synthesis of hydroxyproline of chondrocytes dose-dependently. The optimal concentration of CDGF was 16 ng/ml and 32 ng/ml respectively, and that of IGF-I was 30 ng/ml. There was obvious synergic effect between CDGF and IGF-I.</p><p><b>CONCLUSION</b>CDGF can stimulate the proliferation and collagen synthesis of chondrocytes and has synergistic effect with IGF-I.</p>
